Reporting Safety Incidents

Every member of our college community has the responsibility to create a climate of safety and environmental awareness where the voicing of safety concerns is encouraged.

Call 911 for emergency events where the situation requires immediate assistance from police, fire department or ambulance.

If you need to report a research lab safety event (accident, near miss, concerns), please use the online form linked below. A no-fault investigation will start to determine causes and possible changes to help everyone avoid the problem in the future.

When an injury occurs, first assess the situation to determine if it requires medical or emergency services. If yes, please call 911 immediately.

After assessing the accident and contacting emergency services (if needed), please contact your immediate supervisor and safety officer for your area where the accident occurred.

NOTE: You may not report your own injury/illness. This process must be complete by your supervisor or Human Resource contact.

Supervisors will need to fill our the following two forms:

If an injury is involved but does not require 911 or an ambulance trip to the hospital, be sure the supervisor is notified, and the reports listed above have been completed. All injuries must be reported.

In all cases, once everyone is safe and things are under control, please email the department safety officer and the college safety officer to ensure all necessary reporting was completed.

Reporting hierarchy for concerns or unsafe conditions.

First - Unit Level: Please report any safety related incidents or concerns to your supervisor or local safety officer.
Second - College Level: If you do not feel that your concern or question has been adequately addressed, please reach out to the college' safety officer
Third - University Level: If the local and college contacts do not resolve the issue, please contact EH&S.

If all the above contacts do not address the issue, please feel free to contact OSHA.

Penn State has developed Emergency Action Guides to assist with various types of emergencies. Their informational sheets provide additional guidance for specific types of spills, releases, exposures, or injuries and appropriate recovery and reporting requirements.
